Background
My family has always had dental problems. My wife had her dentures
by age 19. My teeth were bad by age 23, and my son had bad abscesses
and his face would swell horribly, so we know what people go through
with dental problems. I always had a good job and good insurance,
so I could fix my teeth fixed. Forty million + folks have no insurance
and the ones with no dental insurance are much higher than that.
We really enjoy helping people, so we decided one evening that
we would build a Dental Foundation. By surfing the internet and
advice by Attorney Brad Olsen, we created CREATE A SMILE DENTAL
FOUNDATION.
Mission
The mission of CREATE A SMILE DENTAL FOUNDATION is to obtain
funding for dental care for people who have no dental insurance,
inadequate dental insurance, and for those who do not have the
money to obtain dental care, essentially low income individuals,
both children and adults. This will help those who cannot get
help from other sources.
Tax Exempt
We are a 501(c)(3) tax exempt, public funded organization, so
any donations or kind donations are tax deductible. We are registered
with Illinois Attorney General and are a not for profit organization.

How do I Apply to get my Teeth Fixed?
Call the foundation and we will send you the application. We
are building a database of people who need dental work and cannot
afford to pay for it. At present time we are waiting on various
grants to be approved so that we may start creating smiles on
people by fixing their teeth.
Guidelines CASDF
To be eligible for assistance from the foundation:
- The applicant shall have exhausted other sources of assistance,
including but not limited to public aid, Medicare, insurance,
bank accounts, and other sources of assistance.
- If someone has insurance and the individual has to pay a
large amount that the insurance will reimburse them they must
sign a legal document that they will sign the check over to
the foundation when they are reimbursed by insurance of other
means.
- Our income guidelines will be on an individual basis, $1200.00
a month gross qualifies for assistance, if #1 rule is met. Families
add $300 per family member per month. Example: A family of three
with gross income of $1800.00 will qualify.
- We will not pay for braces for pure cosmetic reasons, if
there is medical reason, they will be considered.
- Assistance will be limited to a two county area, Williamson
and Jackson Counties in Illinois, for the present time, until
funds allow us to expand.
- A medical doctor statement that the clients health is being
affected from their teeth problems will cause the person to
move up the list for assistance.
- Dentures will be paid for but will shop around and find the
least expensive place to buy them. There are discount denture
makers and we will utilize those when possible.
- The selection and approval committee will be formed from Board
of Directors and all not related to the founder.
- Guidelines may be added with approval of Board of Directors.
